About the Role:

Panacea Home Healthcare LLC is looking for a ONE-ON-ONE dedicated and compassionate RN or LPN to join our growing team in White Oak, OH. Responsibilities:

• Provide direct patient care in home settings, including assessments, treatments, and medication administration
• Develop, implement, and monitor individualized patient care plans
• Monitor and document patient health status, progress, and changes in condition
• Communicate effectively with physicians, care teams, and family members regarding patient needs
• Educate patients and caregivers on disease management, medications, and home safety
• Maintain accurate and timely clinical documentation in compliance with agency and regulatory standards

Requirements:

• Active RN or LPN license in the state of Ohio (in good standing)
• Prior home health, long-term care, or clinical nursing experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Reliable transportation and valid driver's license for home visits
• Ability to work independently and manage the patient
• CPR/BLS certification required or ability to obtain upon hire
