Grant Funded Position:
This is a grant-funded role with an initial one-year term. Renewal is contingent upon funding and annual review.

Starting Salary:$26.00 per hour

Requirements (Education, Experience, Licensure, Certification):
  • Bachelor’s degree in Criminal Justice, Psychology, Human Services, or related field; OR Bachelor’s degree in any field plus 2 years paid related case management experience; OR High school diploma/GED plus 6 years related service connection or caseload management experience
  • Valid driver’s license

  • Successful completion of extensive background screening process
Preferred Qualifications:
  • Master’s degree in Criminal Justice, Psychology, Human Services, or related field; professional licensure/certification (LSW, LISW, LPC, LPCC, LCDC III); experience working with survivors of human trafficking.
Special Working Conditions and/or Hazards:
  • Employees work indoors and are protected from weather conditions or contaminants, but not necessarily occasional temperature changes.
  • Community based facility, with some exposure to unruly and, at times, out of control populations.
  • Works flexible hours, including days and evenings as needed. Work is subject to frequent interruptions, crisis intervention, constant reprioritization of work activities and a moderate-to-high degree of stress.
Job Duties:
  • Maintains case management and supervision responsibilities for youth participating in the Safe Harbor Diversion Docket.
  • Meets with youth as required and participates in all multidisciplinary team meetings to identify risks and needs.
  • Coordinates with agencies to determine appropriate services and completes referrals to supportive resources that promote successful completion of diversion goals outlined in the Individualized Service Plan.
  • Assists their Manager with responding to referrals and assessing youth using the Ohio Department of Youth Services Human Trafficking Screening Tool.
  • Helps to determine potential eligibility for Safe Harbor diversion.
  • Identifies professionals currently involved with the youth and initiates multidisciplinary team meetings promptly.
  • Collects and reviews collateral information from service providers, community partners, and other relevant individuals to support informed case planning.
  • Prepares clear and accurate court reports summarizing youth progress toward Individualized Service Plan goals.
  • Attends all related court hearings and provides status updates, recommendations, and relevant case information.
  • Serves as liaison among the multidisciplinary team, youth, family, service providers, and the court.
  • Maintains current knowledge of human trafficking trends, legislation, and trauma‑informed practices.
  • Participates in staff meetings, supervision, trainings, quarterly Safe Harbor administrative meetings, and required grant‑related meetings.
  • Completes required quarterly grant reports.
  • Performs other duties as assigned.
  • Demonstrates regular and predictable attendance.
