Medical Social Worker
Garden Park Care Center is a 60 bed Long Term Care Facility seeking a compassionate and dedicated Social Services Director to join our team. This is an excellent opportunity for someone who is passionate about advocating for residents, supporting families, and making a meaningful difference in a skilled nursing and long-term care environment.
What You'll Do:
Coordinate and oversee the facility's Social Services department.
Complete psychosocial assessments and develop individualized care plans.
Assist residents and families with admissions, discharge planning, and transitions of care.
Advocate for residents' rights while promoting their emotional, social, and psychological well-being.
Collaborate with the interdisciplinary team to ensure high-quality resident care.
Provide support and resources to residents and families during difficult situations.
Maintain accurate documentation in accordance with state and federal regulations.
Participate in care conferences, quality improvement initiatives, and survey readiness.
What We're Looking For:
Degree or license in Social Work preferred but not required.
Experience in long-term care or skilled nursing preferred.
Strong communication, organization, and problem-solving skills.
Ability to build positive relationships with residents, families, physicians, and interdisciplinary team members.
Knowledge of state and federal regulations related to long-term care is a plus.
Experience with Point Click Care (PCC) is preferred but not required.
