Social Services Director

Garden Park Care Center is a 60 bed Long Term Care Facility seeking a compassionate and dedicated Social Services Director to join our team. This is an excellent opportunity for someone who is passionate about advocating for residents, supporting families, and making a meaningful difference in a skilled nursing and long-term care environment.

What You'll Do:

• Coordinate and oversee the facility's Social Services department.

• Complete psychosocial assessments and develop individualized care plans.

• Assist residents and families with admissions, discharge planning, and transitions of care.

• Advocate for residents' rights while promoting their emotional, social, and psychological well-being.

• Collaborate with the interdisciplinary team to ensure high-quality resident care.

• Provide support and resources to residents and families during difficult situations.

• Maintain accurate documentation in accordance with state and federal regulations.

• Participate in care conferences, quality improvement initiatives, and survey readiness.

What We're Looking For:

• Degree or license in Social Work preferred but not required.

• Experience in long-term care or skilled nursing preferred.

• Strong communication, organization, and problem-solving skills.

• Ability to build positive relationships with residents, families, physicians, and interdisciplinary team members.

• Knowledge of state and federal regulations related to long-term care is a plus.

• Experience with Point Click Care  (PCC) is preferred but not required.
