Social Services Designee
Garden Park Care Center is looking for a compassionate, organized, and resident-focused Social Services Designee to join our team! If you're passionate about advocating for residents, supporting families, and helping create a positive long-term care experience, we'd love to hear from you.
What You'll Do- Support residents and families throughout their stay
- Assist with admissions, discharges, and care planning
- Coordinate care conferences and communicate with interdisciplinary teams
- Connect residents and families with community resources
- Advocate for residents' rights and emotional well-being
- Maintain accurate documentation and ensure regulatory compliance
- Long-term care or skilled nursing experience preferred
- Previous social services or healthcare experience preferred
- Strong communication and organizational skills
- Compassionate, professional, and resident-centered approach
- LSW a plus, but not required
- Supportive leadership team
