Summary:

Responsible for keeping the treatment team focused on serving the needs of the client. Helps the team engage with the client to meet short and long term goals for discharge and transition to the community.

• Applies interventions focused on the individual’s ability to succeed in the community; to identify and access needed services and to assist the individual with making improvements at home, school and/or in the community.
• Assess client needs on an ongoing basis and determine the appropriate level of care and services needed.
• Provide support and advocacy to clients and families through direct service, case management, crisis de-escalation and/or appropriate therapeutic interventions.
• Assists client in achieving personal independence to manage basic needs and increase daily living skills as identified by the individual, parent/guardian or treatment team.
• Assists clients with identifying and accessing natural and formal community support, services and resources.
• Establish and maintain professional relationships with youth, family and service providing partners.
• Coordinate and assist in crisis management and stabilization as needed.
• Provides interventions to identify and addresses symptoms, behaviors and thought processes to help individuals eliminate barriers to seek or maintain education and employment.
• Develop & coordinate appropriate individualized service plans, reports, discharge and case closure plans in conjunction with treatment support team.
• Document and submit required billable service units for services delivered.
• Initiate services within expected timeframe as set by agency standards.
• Complete progress notes and service documentation within specified timeframe per Lighthouse clinical documentation standards.
• Maintain files in accordance with all regulatory compliance standards.
• Act as liaison between referral sources, external partners and internal LYS service lines and programs.
• Participate in appropriate professional development activities and complete required training.
• Collaborate with all members of the clinical team to ensure quality, continuity, and care for clients.
• Establish and maintain an effective discharge plan from time of intake through discharge.
• Provide after-care supportive services through monthly phone contact for 6 months post discharge.

Supervisory Responsibility:
This position has no supervisory responsibilities.

Community/School Based
- This job operates in a professional office environment, but duties will also be carried out in a variety of settings including client homes, schools and in the community. This role has hybrid work options which involve both remote and in-office work. Specific in-office days will be determined in coordination with the team and the needs of the business. This can change or be altered by the manager at any time.

Shelter Based
- This job operates in a professional office environment and in our satellite offices. This role has hybrid work options which involve both remote and in-office work. Specific in-office days will be determined in coordination with the team and the needs of the business. This can change or be altered by the manager at any time.
