Are you a people person who loves connecting the dots? Do you enjoy solving problems, building relationships, and helping people navigate systems that can sometimes feel like a maze?
We’re looking for a Case Manager who is compassionate, organized, resourceful, and ready to roll up their sleeves to help clients reach their goals.
As a Case Manager, you’ll be the connector between clients, families, treatment teams, schools, community resources, and service providers—keeping everyone moving in the same direction and, most importantly, keeping the client’s needs at the center of the conversation.
What You’ll DoHelp Turn Goals Into Plans
- Work with clients to identify short- and long-term goals and the steps needed to reach them.
- Help clients build the skills and confidence they need to succeed at home, school, work, and in the community.
- Assess client needs on an ongoing basis and help determine the appropriate level of care and services.
- Develop and coordinate individualized service plans with the client, family, and treatment team.
- Keep discharge and transition planning in motion from intake all the way through discharge.
- Connecting clients with community resources, natural supports, and formal services.
- Building strong professional relationships with youth, families, schools, providers, and community partners.
- Serving as a liaison between referral sources, external partners, and internal Lighthouse Youth & Family Services programs.
- Helping clients access the support they need to increase independence and strengthen daily living skills.
- Advocating for clients and families through direct service, case management, crisis support, and appropriate therapeutic interventions.
- Support crisis management and stabilization when needed.
- Help clients identify and address symptoms, behaviors, and thought processes that may create barriers to education or employment.
- Provide mental health prevention services in outpatient and/or school-based settings.
- Help clients and families problem-solve when life gets complicated.
- Stay calm, focused, and trauma-informed when situations become challenging.
- Complete progress notes and service documentation on time.
- Document and submit required billable service units.
- Maintain client files according to regulatory and agency standards.
- Initiate services within required agency time frames.
- Prepare reports, service plans, discharge plans, and case closure documentation.
- Keep information organized and communicate important updates to the treatment team.
- You’ll collaborate with clinical and support teams to make sure clients receive quality, consistent, coordinated care.
- That means communicating, collaborating, attending meetings, completing training, and keeping your professional skills sharp.
- And the relationship doesn’t necessarily end at discharge—you’ll provide after-care supportive services through monthly phone contact for six months following discharge.
- Trauma-informed and able to meet clients where they are.
- A strong communicator, both in conversation and on paper.
- A thoughtful problem-solver and critical thinker.
- Organized enough to keep multiple moving pieces on track.
- Comfortable assessing needs and coordinating services.
- Familiar with health, mental health, and social service systems—or excited to learn them.
- Comfortable using Microsoft Office and case management software.
