What You'll Do

The Population Health Nurse plays a vital role in managing and improving the health outcomes of defined patient populations by using evidence-based practices, data analytics, and community engagement. This role emphasizes prevention, care coordination, and health education to reduce health disparities and improve quality of life. Works in collaboration with primary care providers and all members of the healthcare team. Primary responsibilities include identifying patients appropriate for Annual Wellness Visits (AWV) and Transitions of Care (TOC).

The AWV process includes executing the guided processes and screenings to complete and document the visit. TOC has responsibility for care coordination of our patients who are experiencing a significant change in health status, which has resulted in the necessity of an emergency department visit, inpatient, skilled nursing, or rehabilitative stay. This position requires expertise in the nursing process, using critical thinking skills, planning, and coordinating care.

Responsible and accountable for direct and indirect patient care for designated patient populations as a member of the clinical team. Must be an innovative thinker and open to changing the way we deliver care. Duties may include quality monitoring, patient assessments, direct patient care, and patient/staff education. Provides safe, therapeutic, efficient quality care and services to all patients served. Functions as a member of the health care team at the licensed nurse level to assist in the planning and implementation of patient care, based upon sound nursing judgment, clinical preparation, knowledge, skills, and experience.

Minimum Qualifications

* Graduate of an accredited program in Practical Nursing.

* LPN - Licensed Practical Nurse in the state of Ohio required

* 3+ years as a Licensed Practical Nurse;
Prior experience with care coordination and population management
