Care Coordinator
Location:
Pickaway, OH
Hiring Bonus
:
Eligible for a $3,000 hiring bonus.
Pay Range
: $20.19 - $25.03 per hour (based on experience, education, or licensure).
- Coordinate care for children, youth, and families to identify and fulfill their needs.
- Work with families to recognize cultural factors influencing strengths and interactions.
- Identify strengths to support engagement and positive outcomes.
- Coordinate family‑based services across home, school, and community settings.
- Ensure services on care plans are appropriate, least restrictive, and meet safety and treatment needs.
- Engage and build positive relationships with children, youth, and families in collaboration with child and family teams.
- Develop collaborative partnerships with community resources to meet diverse needs.
- Maintain documentation, participate in program evaluation, attend team and program planning meetings, cross‑systems training, and stay current with community resources.
- Complete necessary training requirements (e.g., High Fidelity Wraparound, MI, Cultural Humility).
- Perform all other duties as assigned.
- Three years of related experience with a high school diploma or equivalent; or two years with an associate or bachelor’s degree; or one year with a master’s degree or higher.
- Knowledge and experience in Hi‑Fidelity Wraparound preferred (certification provided at employment).
- Two years of experience in a coordinated supportive services or care coordination role preferred.
- Experience with people with autism spectrum disorders and developmental disabilities preferred.
- Experience in at least one of the following areas: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, child development.
- Experienced in managing complex cases with cultural humility and responsiveness.
- Able to handle serious emotional disturbance, trauma, co‑occurring behavioral health disorders, and engagement with child‑serving systems (child welfare, intellectual and developmental disabilities, juvenile justice, education).
- Excellent organizational skills; ability to prioritize multiple tasks.
- High degree of cultural awareness.
- Experience with multi‑need individuals and families.
- Broad knowledge of community service systems.
- Willingness to participate in and lead cross‑systems care coordination.
- Effective verbal and written communication skills.
- Ability to operate in an Internet‑based, automated office environment; valid driver’s license required.
- Minimum 100 Mbps high‑speed internet connection required.
