Nurse Practitioner (NP) / Physician Assistant (PA) – Mobile Acute Care
Full-Time | Four 10‑Hour Shifts Per Week
We are seeking experienced Nurse Practitioners and Physician Assistants to join our growing team in Citrus Heights, California. This unique opportunity allows providers to deliver hospital‑level care directly in patients' homes while practicing with greater autonomy, building meaningful patient relationships, and improving healthcare accessibility.
If you have a background in Emergency Medicine, Urgent Care, Internal Medicine, Acute Care, or Hospital Medicine and enjoy working independently while making a direct impact on patient outcomes, we'd love to hear from you.
What You’ll Do- Deliver comprehensive medical care in patients’ homes
- Conduct detailed assessments, examinations, and evaluations
- Diagnose and treat acute and chronic conditions
- Develop individualized treatment plans
- EKGs
- Wound care
- Suturing
- Splinting
- IV therapy and medication administration
- Order and interpret diagnostic testing
- Coordinate follow‑up care and specialty referrals
- Educate patients and caregivers on treatment plans and disease management
- Document patient encounters accurately and thoroughly
- Practice independently while delivering exceptional patient‑centered care
- Four 10‑hour shifts per week
- Every other weekend rotation
- Typical shift options:
- 8:00 AM – 6:00 PM
- 10:00 AM – 8:00 PM
- Localized patient routing to reduce travel time
- Active California Nurse Practitioner or Physician Assistant license
- DEA Registration or ability to obtain upon hire
- Minimum 2 years of APP experience in:
- Urgent Care
- Internal Medicine
- Acute Care
- Experience performing EKGs, catheterizations, and other bedside procedures
- Valid driver’s license
- Home‑based care experience
- Experience practicing autonomously
- Strong acute‑care procedural background
- Comfort managing complex patient populations independently
- Company‑Provided Scrubs
