Join Well Space Health as a Registered Nurse-Care Team member supporting Adult Primary Care providers through patient triage, desktop medicine, care coordination, and collaborative team-based support in a community health center setting.

The Registered Nurse-Care Team supports Adult Primary Care providers and care teams by delivering direct and indirect nursing care in a health center setting. This role assists with desktop medicine activities, including clinical review of patient messages, refill and results workflows, care coordination, and follow-up needs in accordance with established protocols. The RN provides timely triage and nursing assessment for patients who arrive at the health center with urgent or unscheduled needs, prioritizing care based on clinical presentation, psychosocial factors, patient safety, and overall patient flow.

The RN collaborates closely with providers, Medical Assistants, and other support staff to promote efficient team-based care, communicate changes in patient status, and ensure appropriate escalation of symptoms, diagnostic results, or changes in condition. This position requires sound clinical judgment, initiative, flexibility, and the ability to respond quickly and accurately to patient needs with minimal supervision.

• Graduate of an accredited school of Nursing with up to six (6) months Registered Nurse (RN) experience.
• Current Registered Nurse (RN) license within the state of California and BLS certified.
• Previous nursing experience in Emergency Room or Urgent Care experience.
• Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality.
