Employment Type: Contract-to-Hire

Compensation: $56.14/hour

• Monday – Friday, 8:00 AM – 5:00 PM
• Every other weekend on-call (minimal calls/visits)
• Flexibility to have Friday or Monday off during weekend on-call

We are seeking a skilled Registered Nurse (RN) to join our team in Clanton, AL, focusing on comprehensive case management for high-risk patients. The RN will perform detailed assessments, coordinate care transitions, and collaborate with healthcare providers to develop individualized care plans. This role emphasizes both clinical excellence and patient advocacy, ensuring optimal outcomes and quality care.

• Conduct comprehensive assessments of high-risk patients.
• Collaborate with primary care providers to implement individualized care plans.
• Continuously monitor care levels to ensure cost-efficient, appropriate interventions.
• Coordinate non-medical support such as housing and transportation to enhance treatment compliance.
• Facilitate care transitions in collaboration with physicians, social workers, discharge planners, and claims professionals.
• Engage specialty resources to resolve complex patient care needs.
• Maintain detailed records of clinical, functional, and fiscal outcomes.
• Promote health education, illness prevention, and early intervention to prevent adverse events.
• Advocate for patients while ensuring privacy and confidentiality.

• Home Health (HH) experience preferred
• Proficiency with IV therapy, wound vacs, OASIS, ostomies, Foley catheters, and chest tubes

• Current, unrestricted RN license in Alabama
• Associate or Bachelor’s degree in Nursing or related field
• Case Management certification preferred
• Strong analytical and decision-making skills
• Ability to work flexible schedule including minimal weekend calls

• Mileage reimbursement: $0.48/mile
• Counties covered:
  Wilcox, Dallas, Marengo, Clarke
• Contract-to-hire with potential for permanent placement
