Southern Care New Beacon Hospice is seeking an After hours RN Care Manager to provide compassionate hospice care outside standard hours. You will assess and coordinate patient/family needs during nights, weekends, and on-call hours with primary clinical oversight for assigned patients.
You will participate in IDG activities, update plans of care with physicians, educate families, and ensure timely documentation. This role supports regulatory tasks and supervises field staff as needed.
