Position Summary

The Outreach/Case Manager is responsible for engaging and supporting youth experiencing homelessness. This position focuses on community outreach, client engagement, service coordination, and intensive case management to help youth achieve housing stability, personal development, and long‑term self‑sufficiency. The ideal candidate has a passion for working with transitional‑aged youth (TAY), a trauma‑informed approach, and strong knowledge of local resources.

Key Responsibilities
  • Outreach & Engagement
    • Conduct street‑and community‑based outreach to identify and build rapport with youth ages 16–24 experiencing homelessness.
    • Engage youth at drop‑in centers, schools, shelters, encampments, and other high‑need areas.
    • Distribute necessities (e.g., hygiene kits, food, clothing) to support initial engagement.
    • Collaborate with local agencies, probation, and community partners to identify participants.
    • Build and maintain relationships with local shelters, schools, probation officers, mental health providers, and community organizations.
    • Represent the agency at events, meetings, and community collaborations to promote services.
  • Case Management
    • Conduct intakes and needs assessments for new clients.
    • Develop and implement Individual Service Plans (ISPs) based on client goals.
    • Provide ongoing case management, including advocacy, crisis intervention, referrals, and follow‑ups.
    • Assist clients in accessing housing, employment, education, mental health, and substance use services.
    • Monitor client progress and revise service plans as necessary.
  • Documentation & Compliance
    • Maintain accurate and timely case notes, assessments, and records in accordance with program guidelines and confidentiality laws (e.g., HIPAA).
    • Complete required reports and documentation for funders and agency audits.
    • Ensure compliance with all applicable agency, state, and federal regulations.
    • Coordinate with internal staff (e.g., clinicians, housing specialists, life coaches) and external providers to ensure wraparound services.
    • Attend team meetings, trainings, and supervision sessions regularly.
Required Qualifications
  • Bachelor’s Degree in Social Work, Psychology, Sociology, Human Services, or a related field.
  • 1–2 years of experience working in outreach, case management, or related service delivery.
  • Knowledge of trauma‑informed care, harm reduction, and strength‑based approaches.
  • Strong interpersonal, organizational, and communication skills.
  • Ability to work independently and collaboratively in a fast‑paced environment.
  • Proficient in Microsoft Office and case management software.
Preferred Qualifications
  • Experience working with youth, transitional‑aged youth (TAY), or re‑entry populations.
  • Familiarity with local community resources, public benefits, and housing systems.
Work Environment &

Physical Requirements
  • Field‑based and office‑based role with regular community outreach.
  • Must be able to travel locally using personal or agency vehicle.
  • May occasionally work evenings or weekends depending on community events or client needs.
