We are seeking a dedicated Neighborhood/Household Nurse (LVN) to join our team in a on‑call role. In this role, you’ll provide hands‑on nursing care, support a resident‑directed culture, and serve as a teacher and role model for neighborhood/household staff, ensuring high‑quality, individualized care for every resident.

• Administer medications and treatments safely and according to plan of care.
• Observe residents’ physical and emotional condition and report changes to the Nurse Supervisor (RN), Director of Nursing, or attending physician.
• Collaborate closely with the Nurse Leader to ensure regulatory compliance and best practices.
• Support a person‑centered, resident‑directed environment that fosters independence and choice.
• Serve as a mentor and role model for neighborhood/household staff, promoting professional nursing practices.
• Provide routine daily nursing care and services aligned with each resident’s plan of care and personal preferences.
• Advocate for residents’ needs and contribute to a safe, supportive, and homelike environment.

• Graduate of an accredited school of professional nursing.
• Current, active California Licensed Vocational Nurse (LVN) license.
• Minimum of one year of experience in a skilled nursing or senior living setting preferred.
• Ability to read, write, and communicate effectively in English.
• Strong clinical judgment, observation, and documentation skills.
