Summary of Position

This position provides comprehensive school social work services to parents, students and school staff, addressing barriers that limit a student from receiving full benefit from their educational experience and responds to referrals from school administration, parents, teachers, the Student Support Team, the Special Education Committee and others by offering direct services and assisting families in accessing appropriate community resources.

Essential Job Functions
  • Conduct family-centered interviews with the student and family to develop a community-based Plan of Care using the individualized wraparound process and support the child/youth and the family in meeting the objectives of the Plan.
  • Conduct parent interviews to acquire socio-developmental information as needed.
  • Analyze information gained through record reviews and interviews to determine environmental impacts and appropriate resource needs; integrate this information into a written report and oral presentation, and enter case-specific data into the system.
  • Maintain an ongoing liaison with community agencies and other resources to meet student needs; refer parents and students to agencies when appropriate.
  • Serve on school committees as assigned relating to school health, safety and climate.
  • Coordinate and monitor services between agencies to meet the child/youth client and family needs.
  • Develop cooperative working relationships with all members of the Provider Network while ensuring adherence to the Plan of Care.
Education, Certification, and Licensure
  • Education:

    Master’s degree from a recognized college or university with a major study in social work, sociology, psychology, counseling, nursing or a related field.
  • Experience:

    Two years of professional, paraprofessional, or parental advocacy/education experience providing direct service to families, children or youth in social work, psychology, human services, counseling, mental health or equivalent.
  • License/Certification:
    New Hampshire’s valid driver’s license and/or access to transportation with liability insurance coverage as required for frequent travel throughout the state.
Other Knowledge & Skills
  • Strong verbal, writing and computer skills to work with diverse children, youth, families, co-workers, cross-agency stakeholders, and members of the public.
  • Ability to engage individuals from diverse backgrounds and cultures. Bilingual proficiency is desirable. Experience with minority or underserved populations is desirable.
  • Strong knowledge of public benefit, mental health, child welfare and education systems.
  • Commitment to system of care core values.
  • Ability to maintain strict confidentiality guidelines.
  • Must be organized and able to work well under pressure, meeting deadlines with efficiency and consistency while maintaining job requirements.
