- Monitor residents mental and physical well-being, notify supervisors of changes, and document observations accurately
- Receive, transcribe, and implement physician orders; administer medications and treatments
- Supervise and support nursing assistants in providing quality resident care
- Assist with wound care, oxygen administration, gastrostomy/jejunostomy feedings, and infection control practices
- Aid in resident admissions, discharges, and transfers with proper documentation
- Maintain communication with residents families and coordinate care with interdisciplinary teams
- Manage unit operations during shifts, ensuring compliance with regulations and safety standards
- Support staff orientation and ongoing education to maintain high-quality care standards
Required:
- Valid LPN/LVN license in New Hampshire
- Minimum of 1 year of nursing experience, preferably in long-term care
- Ability to work independently and as part of a team
- Excellent communication and critical thinking skills
- Own transportation due to location in a rural area with limited public transit
- Experience with wound vacs and medication management
- Knowledge of infection control and safety regulations
