Licensed Vocational Nurse (LVN) / Licensed Practical Nurse

This position is located in a rural area with limited housing options and public transportation. Candidates must have their own transportation. The LVN/LPN will observe each resident consistently, being alert to changes in their mental and physical well-being. The role includes notifying the Supervisor of pertinent observations and discussing the need to notify a physician. Accurate receipt, transcription, and execution of physician's orders are required.

Medications and treatments must be administered as ordered by the physician, with proper documentation via a computerized system. The LVN/LPN will order and maintain pharmaceuticals for the units. Supervision of LNA staff in accordance with licensing requirements is necessary. Tasks include assigning and measuring nursing staff resident care consistent with abilities and supervising care given. Assisting direct-care staff with nursing care as necessary and as time allows is expected.

The LVN/LPN will accompany the physician on resident rounds to address medical needs and facilitate communication. Oxygen administration must follow facility guidelines and procedures. Gastrostomy/jejunostomy feedings must be administered per policy and procedure. Wound care and knowledge of wound vacs are required. Regular duties include checking appointments, activity boards, and reminding staff of various tasks. Monitoring LNA documentation and updating LNA records is essential.

Oversight of infection control practices and ensuring a clean, hazard-free unit is required. Reporting any issues to the Nurse Manager or Nursing Supervisor is expected. Effective communication with residents' families to resolve concerns and notify them of orders is crucial. Managing assigned units under the direction of the Nursing Supervisor is part of the role. The LVN/LPN must follow State and Federal regulations regarding controlled medications.

Assisting with orientation of new nursing employees is expected. Communication regarding staff education needs and assisting in training as needed is part of the job. Evaluating assigned nursing staff members according to facility policy is required. Initiating or recommending disciplinary action as necessary may be part of the role.
