Licensed Vocational Nurse (LVN/LPN)
Seeking a licensed vocational nurse (LVN/LPN) to provide nursing care in a rural long-term care/skilled nursing facility. The role involves supervising staff, administering medications, and coordinating resident care to ensure quality outcomes.
Responsibilities:
- Observe and assess residents' mental and physical health, reporting any significant changes to the supervisor.
- Administer medications and treatments in accordance with physician's orders, accurately documenting in electronic systems.
- Supervise and coordinate with Licensed Nursing Assistants (LNAs), ensuring compliance with licensing standards and scope of practice.
- Assist physicians during resident rounds to facilitate communication and address medical needs.
- Ensure infection control practices are maintained, and the unit remains clean and hazard-free.
- Manage admissions, discharges, and transfers, completing all necessary documentation and notifications.
Qualifications:
- Hold an active LVN/LPN license in the state of New Hampshire.
- Have a minimum of 3 years of recent (within the last 6 months) LVN/LPN experience in a long-term care or skilled nursing setting.
- Be IV certified or willing to obtain certification during onboarding.
- Demonstrate CPR/BLS certification at the time of application.
- Possess reliable transportation and reside within 50 miles of the facility.
- Be willing to float to other units as needed and comply with all vaccination requirements, including COVID-19 vaccination with proof.
