LVN / LPN Position

The LVN / LPN provides skilled nursing care to residents in a long-term care or skilled nursing facility setting. This role involves monitoring residents’ health statuses, administering medications, and collaborating with the healthcare team to ensure quality resident care.

Responsibilities:

  • Observe and assess residents’ mental and physical well-being consistently throughout shifts.
  • Administer medications and treatments accurately following physician orders.
  • Document all care and medication administration in electronic health records, including MARs/TARs.
  • Supervise and support nursing assistants, ensuring compliance with licensing requirements.
  • Assist during physician rounds and facilitate communication regarding residents’ medical needs.
  • Support infection control practices, maintain unit cleanliness, and report hazards or issues promptly.

Qualifications:

  • Valid state license as LVN or LPN with active certification.
  • Minimum of 3 years recent experience in a long-term care or skilled nursing setting.
  • Willingness to float to different units based on staffing needs.
  • IV certification or willingness to obtain during onboarding.
  • Proof of COVID-19 vaccination required; exemptions are not accepted.
  • Ability to work independently and as part of a team in a rural facility environment.
