Job Summary:

Seeking a licensed practical nurse (LPN) to provide high-quality nursing care in a rural skilled nursing facility. The role involves comprehensive patient assessments, medication administration, and supervision of CNA staff to ensure optimal resident outcomes.

Responsibilities:

  • Observe and document residents’ mental and physical well-being, escalating changes to the Supervisor and physician as needed.
  • Administer medications and treatments accurately according to physician orders, maintaining electronic documentation.
  • Supervise and assign duties to Licensed Nursing Assistants (LNAs) in compliance with state licensing requirements.
  • Assist during physician rounds, address residents’ medical needs, and facilitate effective communication.
  • Perform wound care, including managing wound vacs, and provide other clinical procedures per policies.
  • Maintain unit cleanliness, monitor infection control practices, and ensure safety protocols are followed.

Qualifications:

  • Hold an active LVN/LPN license in good standing.
  • Demonstrate a minimum of 3 years recent clinical experience within the last 6 months.
  • Be willing to float to other units as needed.
  • Obtain IV certification (willing to get during onboarding if not certified).
  • Be Covid vaccinated with proof of vaccination; no declinations accepted.
  • Reside more than 50 miles from the facility location.
