LVN​/LPN – LTC​/SNF

Job Summary:

The Licensed Practical Nurse (LPN) provides comprehensive nursing care to residents in a rural healthcare facility, ensuring their physical and mental well-being. This role involves medication administration, patient assessments, and supervision of nursing staff to deliver quality care.

Responsibilities:

• Observe and monitor residents’ health status, reporting changes to the Supervisor and discussing necessary notifications.
• Accurately transcribe and execute physician’s orders, including medication and treatment administration.
• Supervise LNAs, assign care tasks, and assist with direct care as needed, ensuring compliance with licensing standards.
• Assist with resident rounds alongside physicians and coordinate communication during 60-day reviews.
• Administer oxygen, gastrostomy/jejunostomy feedings, and perform wound care following facility protocols.
• Manage unit documentation, oversee infection control practices, and maintain a safe, hazard-free environment.

Qualifications:

• Active Licensed Practical Nurse (LPN) license in New Hampshire.
• Minimum of 3 years recent nursing experience, with experience in rural/small facilities highly preferred.
• Certification in BLS (AHA); IV certification required or willing to obtain during onboarding.
• Willingness to float across units and work extended shifts as necessary.
• Ability to work independently, lead a team, and communicate effectively with residents, families, and staff.
• Must have reliable transportation and reside more than 50 miles from the facility.
