Position: LVN / LPN – LTC/SNF

Call for Salary Rate –

Job Summary:

Provide compassionate and skilled nursing care to residents in a rural LTC/SNF facility, ensuring their physical and emotional needs are met with vigilance and professionalism.

Responsibilities:

  • Observe and document residents’ mental and physical status, communicating changes promptly to the Supervisor and medical team.
  • Administer medications, treatments, and oxygen therapy accurately according to physician’s orders and facility policies.
  • Supervise and support nursing staff and LNAs, ensuring compliance with licensing requirements and care standards.
  • Assist physicians during rounds and facilitate communication regarding residents’ medical needs and care plans.
  • Manage unit operations, including resident admissions, discharges, and transfers, while maintaining proper documentation and reporting.
  • Monitor infection control practices, ensure a hazard-free environment, and oversee medication storage and documentation.

Qualifications:

  • Valid New Hampshire RN license or LVN/LPN license, active at the time of employment.
  • Minimum of 3 years recent nursing experience within the last 6 months, preferably in a LTC or small facility setting.
  • Current BLS certification; IV certification required or willingness to obtain during onboarding.
  • Willingness to float to other units as needed and to work in rural, limited-transportation environments.
  • Commitment to resident safety, quality care, and regulatory compliance.
  • COVID-19 vaccination required; declinations are not accepted.
