Position: LPN / LVN - LTC

Licensed Vocational Nurse (LVN)

This position is in a rural area with limited housing options and public transportation. Candidates must have their own transportation. Responsibilities include observing residents for changes in mental and physical well-being. Notify the Supervisor of pertinent observations and discuss the need to notify the physician. Receive and provide a detailed report of residents' status during shift changes. Accurately receive, transcribe, and carry out physician's orders.

Administer medications and treatments as ordered and ensure proper documentation. Order and maintain all pharmaceuticals for the units. Supervise nursing assistants in accordance with licensing requirements. Assign and measure nursing staff resident care consistent with abilities and job description. Accompany the physician on resident rounds to address medical needs. Administer oxygen and perform gastrostomy/jejunostomy feedings as per policy. Perform wound care and have knowledge of wound vacs.

Check daily appointments for therapy sessions and remind staff of essential tasks. Monitor nursing assistant documentation and ensure infection control practices. Admit, discharge, and transfer residents, completing necessary documentation. Complete 24-hour reports for the assigned units, check new orders, and maintain communication with nursing management. Communicate with residents' families to address concerns and notify them of new orders. Determine appropriate responses to incoming calls.

Manage assigned units during shifts under the direction of nursing leadership. Follow regulations regarding controlled medications and notify supervisors of discrepancies. Assist with the orientation of new nursing employees. Communicate continuing education needs and assist in training delivery. Encourage attendance at in-service education meetings. Evaluate assigned nursing staff members according to facility policy. Initiate or recommend disciplinary action as necessary.
