Looking to make a meaningful difference in people's lives? We are seeking a compassionate and organized Services Coordinator (Self-Directed Services) to join our team in the Claremont, NH area. In this full-time role, you'll work closely with individuals and families receiving developmental and community-based services, helping them navigate resources, develop personalized support plans, and build greater independence. This position offers some schedule flexibility, local travel, and an on-call rotation, making it perfect for someone who enjoys helping others and working independently.

- Manage and coordinate services for individuals and families, ensuring personalized support plans are implemented effectively
- Develop and monitor individual service budgets
- Build strong relationships with clients, caregivers, providers, and community partners
- Assist with resource navigation, including Medicaid, Medicare, Social Security, and local programs
- Prepare necessary documentation, service agreements, and reports
- Participate in team meetings and offer solutions to service-related challenges
- Travel locally to meet with clients and collaborate with support networks
- Participate in an on-call rotation for caseload coverage
Qualifications:
- Bachelor’s degree in Human Services, Social Work, Psychology, Sociology, Behavioral Health, or related field (or relevant experience)
- Minimum of 1 year of experience in case management, developmental services, or community-based support
- Excellent communication and interpersonal skills
- Strong organizational, documentation, and time-management abilities
- Ability to work independently and manage multiple priorities
- Basic understanding of public benefits and community resources
- Valid driver’s license and reliable transportation
- Ability to pass background and employment screenings
- Competitive pay ranging from $22 to $23 per hour
