Physical Therapy - PTA SNF

Seeking a dedicated Physical Therapist Assistant (PTA) for a full-time permanent Skilled Nursing Facility (SNF) position. The PTA will work Monday through Friday (8:00 AM–4:30 PM, 40 hours per week) providing rehabilitative services under the supervision of a Physical Therapist to help residents improve mobility, strength, balance, and functional independence. This opportunity serves a 300-bed Veterans population, offers no weekend rotation, and provides leadership growth potential, with prior Director of Rehabilitation (DOR) experience highly preferred.

Salary range is $83,000–$94,000 annually.

Requirements:

• 1+ year of PTA experience required
• Skilled Nursing Facility (SNF) or Rehabilitation experience required
• Active PTA license required
• Experience treating geriatric and long-term care patients preferred
• Prior Director of Rehabilitation (DOR) experience highly preferred
• Strong communication and interdisciplinary collaboration skills required
• Knowledge of therapeutic exercises, gait training, and mobility interventions required
• Ability to document accurately in an electronic medical record required
• New graduates will not be considered

Responsibilities include implementing treatment plans established by the Physical Therapist, providing therapeutic exercises, gait and transfer training, balance and strengthening activities, mobility interventions, use of assistive devices, documenting patient progress, communicating treatment outcomes to the supervising therapist, participating in interdisciplinary care meetings, assisting with discharge planning, educating patients and caregivers, and delivering high-quality rehabilitation services to a veteran population in a skilled nursing setting.
