Job SummaryAs a Licensed Practical Nurse (LPN) at Emerald Care Center Claremore, you will play a crucial role in delivering high-quality, compassionate care to our residents. Your primary responsibilities will include assessing patient needs, providing direct nursing care, and collaborating with the healthcare team to ensure the best outcomes for our residents. You will work in a supportive and dynamic environment where your skills and dedication contribute significantly to the well‑being of our residents.
WorkEnvironment
Work is performed in a skilled nursing facility with potential exposure to infectious diseases and hazardous materials. Appropriate personal protective equipment (PPE) is provided.
Responsibilities- Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed.
- Monitor residents’ vital signs, document progress, and report significant changes to the Charge Nurse or Director of Nursing.
- Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident’s needs.
- Communicate effectively with residents, families, and the healthcare team, maintaining clear and accurate documentation.
- Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols.
- Provide emotional support and education to residents and their families, helping them understand care plans and health conditions.
- Collaborate with other healthcare professionals—RNs, CNAs, physicians, and therapists—to deliver coordinated and comprehensive care.
- Valid LPN/LVN license in Oklahoma.
- Graduation from an accredited LPN/LVN program.
- Preferred:
Previous experience in a skilled nursing facility or similar healthcare setting. - Current CPR/BLS certification required. Additional certifications may be required based on state regulations.
- Excellent verbal and written communication skills. Ability to work collaboratively with a diverse team of healthcare professionals and build positive relationships with residents and their families.
