Job Description

Registered Nurse, Ambulatory who uses the nursing process to assess, plan, implement, and evaluate patient care. Manages patient teaching, communication, triage, and documentation in Epic, ensuring patient safety, privacy, and timely response to clinical needs. Collaborates with the care team, supports quality improvement, promotes evidence‑based practice, and upholds PRIDE values and organizational goals.

  • Utilize the nursing process effectively (assessing the patient and situation, identifying the problem, planning, implementing, and evaluating care through the health care continuum).
  • Consistently demonstrate competent clinical skills and coordinate patient assessment and re‑assessment to positively impact quality outcomes.
  • Assess barriers to learning and functional status of patient; coordinate, evaluate, and complete patient/family teaching.
  • Respond to patients and families in a courteous, positive, and timely manner.
  • Respect patients’ right to privacy, dignity, and confidentiality per HIPAA; foster a positive environment that promotes PRIDE values.
  • Promote better patient outcomes by reviewing open gaps of care and coordinating closure of care gaps; interact with patients and team members, document in Epic, and facilitate interdisciplinary collaboration.
  • Manage in‑basket, pharmacy contact, medication ordering/renewal; review and disseminate labs based on acuity; respond to patient messages with clinicians.
  • Recognize emergency situations, clarify physician orders, maintain safe environment for team and patients; utilize HRO safety principles.
  • Report all injuries (patient or staff) using the event reporting system.
  • Administer medications and vaccines according to policy; carry out physician orders accurately and timely; monitor effects and report adverse reactions.
  • Know policies, procedures, and emergency equipment; identify and report risk management issues promptly.
  • Triage phone calls using critical thinking to evaluate chief complaint and urgency; forward to appropriate clinician and document all communications.
  • Participate in practice huddles; communicate effectively, listen, seek feedback, and resolve conflicts; act as mentor and resource to team.
  • Follow standard precautions, isolation techniques, and Ambulatory Health Care National Patient Safety Goals.
  • Collaborate via Shared Governance to promote evidence‑based care within a healing culture.
  • Document patient responses accurately; verify order entry and transcription; maintain competency in information systems.
  • Pursue formal and independent learning to enhance knowledge, competency, and professional development.
  • Monitor quality dashboards for assigned clinicians/practice.
  • Perform other duties per operational needs within scope of RN license.
  • Support overall strategic goals of the clinical practice area, AMG, AHS hospital sites, Atlantic Visiting Nurse, and system.

Qualifications
  • High School Diploma or GED.
  • Graduate of an accredited School of Nursing.
  • RN license, valid in the State of New Jersey.
Preferred
  • At least 2 years of clinical ambulatory practice.
  • Bachelor’s degree in Nursing.
  • American Heart Association Basic Life Support (BLS) credential preferred.
