The CLC RN collaborates with the Minimum Date Set (MDS) coordinator to ensure documentation is accurate and reflective of the Resource Utilization Group (RUG) score.
The CLC RN provides care to residents with chronic medical, as well as surgical diagnoses. Many of these residents often have complex diagnosis requiring skilled nursing assessment, monitoring, IV infusions, wound care, and many other needs. He/she provides guidance and education to patients and caregivers and collaborates with other professionals at all levels, including but not limited to:
Social Workers, Pharmacists, Radiology Staff, Laboratory Staff and the other Nurses and Physicians on the various clinical treatment teams. Collaborates with the CLC interdisciplinary team to promote resident-centered care and provide a home like environment for long term care and hospice/palliative care residents. The CLC RN is competent in identifying physiological, psychological, social, and environmental variable affecting the functional status of the older adult and hospice/palliative care patient and anticipates possible discharge needs and assists patient/significant others and multidisciplinary team in achieving goals for discharge.
Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs. Responsibilities may also include charge nurse duties, medication administration, documentation, and assistance with ADLs.
