Minimum Qualifications

  • Current Licensed Practical Nurse license issued by the state in which services will be provided or current multi-state Licensed Practical Nurse license through the enhanced Nurse Licensure Compact (eNLC).
  • Valid driver’s license.
  • CPR certification within 30 days of hire date.

  • One (1) year of Hospice Nursing experience.
Core Duties and Responsibilities
  • Perform and monitor basic patient care in the patient’s home in accordance with policies and procedures.
  • Use the nursing process (assessment, planning, implementation, evaluation) to meet the patient’s physical and psychological needs.
  • Provide a safe, comfortable environment and ensure privacy, dignity, and confidentiality of all patients.
  • Complete assignments timely and communicate patient care problems or potential problems to the primary RN, administrative staff, and the interdisciplinary team.
  • Document a full system assessment, including physical and psychosocial aspects, and update the care plan every visit.
  • Implement and update the nursing problem list in collaboration with the RN.
  • Report significant changes in patient condition to the RN and administrative staff or physician.
  • Follow the plan of care based on assessed findings and update under physician orders and the RN’s guidance.
  • Document patient status and care services furnished in appropriate areas as defined by the documentation policy and procedure.
  • Perform CPR, bag technique, and observe and notify RN/administrative staff of skin changes, infiltration, phlebitis, infections, circulatory overload, and allergic reactions.
  • Perform treatments and procedures: vital signs, patient weights, specimen collection, ostomy care, urethral catheterization, pressure wound care, dressing changes, oral suction, range‑of‑motion exercises, bladder training, bowel training, ambulation/transfer, pacemaker management, and diabetes care.
  • Provide patient teaching, anticipate and modify the teaching plan, and evaluate its effectiveness regularly.
  • Coordinate medication administration in compliance with the Medication Administration Standards and report discrepancies to the RN and administrative staff.
  • Facilitate changes in level of care in compliance with the change‑in‑care standard and report changes to the RN and staff.
  • Understand stages of death and dying and provide appropriate hospice care for terminally ill and acutely ill patients.
  • Perform post‑mortem care as needed.
  • Execute technical duties: IV therapy excluding pump removal, wound vacs, sterile wound care, pleurx drain care, ostomy care, trach care, G‑tube care, Foley and suprapubic catheter care, laboratory access, device changes, and comprehensive head‑to‑toe assessments.
Physical Requirements
  • Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, and lifting required during the shift.
  • Ability to lift, move, and position patients of all weights with varying assistance.
  • Normal hearing and visual acuity to assess breath sounds, bowel sounds, apical pulse, and monitor equipment.
  • Occasional exertion of 20–50 pounds of force and frequent exertion of 10–25 pounds of force; sometimes up to 10 pounds of force constantly.
  • Must be able to operate a motor vehicle safely.
Working Environment
  • Exposure to bodily fluids, human tissues, contagious diseases, sharps, and explosive gases.
  • Exposure to toxic gases, fumes, odors, electrical current, and radiation from X‑rays.
  • Exposure to high stress, constant interruptions, bad road conditions, wind, rain, snow, sleet, mud, hazardous roadways, and unpredictable home environments.
  • Predominantly performed in the patient’s home with extensive travel required.
Skills and Abilities
  • Competency in technical and clinical skills, completing a skills checklist and performing required skills daily.
  • Ability to administer medications via all acceptable routes as ordered by a physician and authorized by the RN.
  • Strong written and verbal communication skills for effective multidisciplinary collaboration.
  • Mathematical skills to calculate drug dosages and infusion rates.
  • Knowledge of patient rights and compliance with safety policies and Federal, State, and local regulations.
