Seven Healthcare are seeking a dedicated RN – Home Health – Travel professional to join our team in Clarksburg, WV. This contract position requires providing exceptional home health services within defined counties, ensuring the effective implementation of care plans and disease prevention strategies. The role involves both independent work and collaboration with a multidisciplinary team.

Key responsibilities of the RN – Home Health – Travel include:
- Observe, assess, plan, implement, and evaluate nursing support in a community or small facility setting.
- Work closely with health professionals/providers to plan and evaluate programs to address health needs.
- Coordinate communications and actions of various disciplines regarding health and safety.
- Order, review, monitor, and manage medications as prescribed by a physician.
- Ensure policies for medication administration and errors are in place and reviewed annually.
- Report progress to the physician when client conditions change.
- Transcribe or ensure transcription of physician orders and medically related reports.
- Complete required nursing documentation.
- Document individuals' health status in nurse's notes routinely.
- Maintain equipment and supplies to prevent disease spread.
- Direct and supervise licensed practical/vocational nurses.
- Schedule and complete health-related appointments and implement findings.
- Maintain communication with inpatient staff during hospitalization of persons served.
- Provide training for health and safety-related topics.
- Ensure unlicensed staff are trained on health maintenance, safety, and disease prevention.
Requirements of the RN – Home Health – Travel include:
- WV RN license required
- Home Health Experience required
- Proficiency in HCHB and OASIS required
