Key Responsibilities of the LPN / LVN

• Provide care for clients following their plan of care while maintaining dignity and independence
• Monitor client's vital statistics and report abnormal findings
• Administer medications, feedings, oxygen, ostomy care as needed
• Complete routines established by other care professionals such as PT, OT, Speech Therapists
• Ensure infection control policies are maintained and report communicable diseases or infections
• Accurately document nursing actions and progress towards established goals
• Order necessary supplies and medications to ensure adequate amounts are available
• Ensure patient needs align with physician's instructions
• Monitor and report incidents, ensuring timely documentation and notification
• Maintain medical equipment and report issues for repair

Requirements of the LPN / LVN

• WV LPN license
• Home health experience
