Seven Healthcare is seeking a skilled LPN / LVN to join a dedicated team in Clarksburg, WV. This travel contract offers the opportunity to work in a dynamic home health environment, providing care and support to clients while ensuring their dignity and independence.

Key Responsibilities of the LPN / LVN:

• Provide care for clients following client's plan of care at all times while maintaining client's dignity and independence.
• Monitor client's vital statistics and report abnormal findings.
• Administer medications, feedings, oxygen, and ostomy care as needed per client's plan of care and provide assessments of results.
• Complete routines as established by other care professionals such as PT, OT, Speech Therapists.
• Ensure infection control policies are maintained at all times and report possible communicable diseases or infections.
• Document nursing actions of all care given and communication as appropriate.
• Order supplies, medications, and feedings as needed to ensure adequate amounts are on hand at all times.
• Ensure all patient needs are being met in accordance with the instructions of the physician.
• Monitor incidents involving individuals and ensure incident reports are completed timely and accurately.
• Ensure all medical equipment is maintained and report faulty equipment to DME for repair.

Requirements of the LPN / LVN:

• Valid WV LPN license
• Home health experience
• Experience with HCHB
