Admissions Nurse; RN Elite Home Health & Hospice | LC Valley Area

What You’ll Do

• Complete an initial, comprehensive and ongoing assessments to determine home health and/or hospice needs.
• Develop patient focused care plans that establish goals, based on nursing diagnosis and incorporates palliative nursing actions.
• Provide respectful, compassionate care in patients’ homes.
• Act as Case Manager and assume responsibility to coordinate patient care for an assigned caseload.
• Collaborate with interdisciplinary team members.
• Communicate with the physician regarding the patient’s needs and report changes in the patient’s condition; obtain/receive physician’s orders as required.

• Active RN licensure (state-licensed).
• Valid driver’s license, reliable transportation, and current auto insurance.
• Strong communication skills and a team-oriented mindset.
• Ability to work independently in home settings.
• Commitment to patient-focused care.

Wage: $38 - $40 per hour
