Admissions RN- Hospice

TN Quality Care Hospice is seeking an Admissions RN to join their growing team in the Clarksville area. This field-based position is responsible for performing admission assessments and initiating a plan of care that optimizes patient and family-directed comfort and quality of life.

HOURS:
Monday-Friday 8:00am-4:30pm

TERRITORY:
Clarksville and the surrounding area.

Qualificatons:

• Graduate from an accredited registered nursing program
• Licensed as a registered nurse in the state of practice
• Must possess current CPR Certification
• 1 year of Hospice experience, preferred
• Must possess the ability to make independent decisions when circumstances warrant
• Must possess the ability to deal tactfully with patients, family members, visitors, agency personnel, and the general public
• Must be knowledgeable of quality assessment and assurance procedures

What You’ll Do:

• Perform pre-admission evaluations for the appropriateness of services.
• Initiates the admission process by educating and informing patients/families of its services.
• Obtains informed consent
• Provides nursing assessment, initial plan of care, and intervention(s) that optimize patient and family-directed comfort and quality of life.
