Job Summary
The Advanced Practice Clinician (NP/PA) is service/physician aligned, managing a distinct patient population. The APC may be required to perform outpatient and inpatient care and facilitate discharges/outpatient follow‑up. The APC demonstrates advanced medical and nursing knowledge, clinical competence, sound judgment, professional communication, compliant documentation, and is responsible for professional development and competency validation.
Key Responsibilities- Assessment of health status, perform and document a complete history and physical examination for acute and complex chronically ill patients.
- Order and collect data using appropriate assessment techniques, supporting diagnostics, and diagnostic procedures where indicated.
- Serve as first responder for both nursing and staff in the event of patient status changes, orders, or situations requiring procedural and resuscitative interventions.
- Assess risks associated with care of the acute and complex chronically ill patient including medication side effects, immobility, nutrition, fluid and electrolyte imbalance, immunocompetence, and invasive interventions.
- Coordinate daily clinic schedule with attending physician team, present patient reports, collaborate in plan of care, and follow assigned patient population.
- Formulate individual, dynamic plan of care utilizing evidence‑based practice guidelines or protocols across the continuum.
- Implement and modify plan of care to achieve expected outcomes.
- Prescribe diagnostic strategies and therapeutic interventions – pharmacologic, non‑pharmacologic, and surgical – needed to achieve expected outcomes.
- Perform advanced procedures consistent with privileges and competency validation, including minor suturing, complex wound management, line and tube placements and removals.
- Maintain ongoing communication and collaboration with the interdisciplinary health care team.
- Document key components of patient progress via daily progress notes, transfer and discharge summaries, or clinic notes as applicable; ensure documentation is timely, meets compliance standards, and captures patient acuity.
- Demonstrate professional practice behaviors including precepting, mentoring, education and instruction of students, nursing staff, and new APCs.
- Seek opportunities for active engagement in research and analysis of evidence‑based practice; participate in Grand Rounds, APN council meetings, faculty and unit meetings, and M&M presentations.
- Maintain CEUs and membership in a professional organization.
Certifications: Lic‑Licensed Nurse Practitioner – Tennessee, Lic‑Physician’s Assistant Certified – Tennessee
Work Experience: Relevant work experience managing acute and complex chronically ill patients.
Experience Level: 1 year
Education: Master’s degree in nursing or related field.
