Responsible for administering and directing appropriate nursing interventions as prescribed by a qualified physician or mid-level practitioner. Responsible for effective communication and accurate documentation. Displays professional behavior in accordance with Nursing Code of Ethics and CHS philosophy of service.

Full-time (40 hours/week), Nights

• Perform the initial nursing assessment and communicate findings with the assigned physician or mid-level practitioner.
• Assess wound stage and measurements, document care plans, and evaluate patient status in regards to desired outcomes.
• Identify patient care situations that require intervention.
• Participate in quality improvement initiatives, provide education to the patient and/or caregiver, and serve as patient advocate in the delivery and coordination of care.
• Collaborate as needed with other healthcare providers, Wound Care Center Providers, the Program Director and Medical Director regarding patient needs.
• Perform other duties as required.

• Registered Nurse licensed (LIC-Registered Nurse).
• Relevant work experience: 2 years.
• Graduate of an approved discipline-specific program.
