Job Summary
The Advanced Practice Clinician (NP/PA) is a service/physician‑aligned provider who manages a distinct patient population. The role includes outpatient and inpatient care, discharges, and follow‑up while ensuring adherence to clinical pathways and protocols. The Clinician demonstrates advanced medical and nursing knowledge, professional communication, and documentation standards while supporting professional development and competency validation.
Key Responsibilities- Assesses patient health status by completing a full history and physical examination for acute and complex chronically ill patients.
- Orders and collects data using appropriate assessment techniques, diagnostic tests, and procedures where indicated.
- Serves as the first responder for both nursing and staff in the event of patient status changes, including procedural and resuscitative interventions.
- Identifies risks associated with care of acute and chronic patients, including medication side effects and immobility.
- Coordinates daily clinic schedules with the attending physician team, presents patient reports, collaborates in plan of care, and follows assigned patient populations.
- Self‑forms and documents individualized plans of care based on evidence‑based practice guidelines to address complex acute and chronic health needs.
- Implements and modifies plans of care, prescribing diagnostic strategies and therapeutic interventions, both pharmacologic and non‑pharmacologic.
- Manages further diagnostic tests through ordering, interpretation, performance, and supervision.
- Performs advanced procedures consistent with privileges and competency validation, including minor suturing, complex wound management, line and tube placements and removals.
- Maintains ongoing communication and collaboration with an interdisciplinary health‑care team, and facilitates communication with patient, family, and staff to promote comprehensive care.
- Documents and dictates key patient progress via daily progress notes, transfer and discharge summaries, and clinic notes in the medical record.
- Demonstrates professional practice behaviors, including precepting, mentoring, and education of students, nurses, and novice APCs.
- Seeks opportunities for active engagement in research and evidence‑based practice, and participates in grand rounds, council meetings, faculty and unit meetings, and M&M presentations.
- Maintains continuing education units (CEUs) and membership in a professional organization.
- Certification:
Licensed Nurse Practitioner (Tennessee) or Licensed Physician Assistant (Tennessee). - Education:
Master’s degree in an approved field. - Work Experience:
Minimum 1 year relevant experience in advanced practice clinical care.
