LTSS Service Coordinator - Clinician
Shelby, Dickson, Humphreys, Knox County, and surrounding counties.
Location: This field-based role enables associates to primarily operate in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training. Alternates may be considered if candidates reside within commuting distance of an office. Candidates not within a reasonable commuting distance will not be considered unless an accommodation is granted as required by law.
The LTSS Service Coordinator–Clinician is responsible for the member’s case and contributes to the LTSS care coordination process by performing activities within the scope of licensure, including assisting the responsible RN with assessments, identifying, evaluating, coordinating, and managing member needs across physical, behavioral, social, and long‑term services.
How You Will Make An Impact- Assists in identifying members for high‑risk complications. Obtains clinical data as directed.
- Assists in identifying members that would benefit from an alternative level of care or other waiver programs.
- Provides all collected information to the responsible RN, who verifies and interprets the data, conducts additional assessments, as necessary, and develops, monitors, evaluates, and revises the member’s care plan.
- Participates in coordinating care for members with chronic illnesses, co‑morbidities, and/or disabilities, ensuring cost‑effective and efficient utilization of health benefits.
- Decision‑making is based on current member needs, requiring understanding of disease processes, terminology, and clinical guidelines, but does not require nursing judgment.
- License: LPN/LVN, LSW, LCSW, LMSW, or other applicable state license (not RN). Must be current and unrestricted in the applicable state(s).
- Education:
Nursing diploma, AS in Nursing, or related field, plus a minimum of 2 years of experience working with individuals with chronic illnesses, co‑morbidities, and/or disabilities in a Service Coordinator or similar role, or an equivalent combination of education and experience.
- Strong preference for case management experience with older adults or individuals with disabilities.
- BA/BS in Health or Nursing preferred.
