We are seeking a Certified Home Health Registered Nurse (RN) with at least 1 year of home health experience and proficiency in OASIS
.
In this role, you will provide independent, high-quality care in patients' homes
, conducting comprehensive assessments and developing personalized care plans.
Timely documentation of care using Home Care Home Base EMR and ensuring patient needs are met effectively.
Qualifications- Current, unencumbered RN license.
- Wound care skills (including wound vac).
- Reliable transportation.
- Current CPR certification.
- Preferred certifications in home health or geriatrics.
