Job Summary and Qualifications

As a Registered Nurse for Admission and Discharge, the top priority is delivering exceptional patient‑centered care. I am dedicated to ensuring a seamless and efficient transition for patients coming in and out of acute care hospital settings. Specialized assessment skills help ensure that patients are placed in the appropriate care areas and discharged. Education and post‑hospitalization care are offered as necessary.

The main focus is advocating for patients, families, and caregivers while embodying the organization’s vision, mission, and values, to give an unmatched patient experience and attain optimal clinical outcomes.

What Will You Do in This Role
  • Obtain a nursing assessment history and ensure proper inpatient care unit admission. Take appropriate action when needed, such as communicating special circumstances to physicians and/or care team or reporting indications of abuse or neglect.
  • Complete and facilitate the completion of required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post‑hospital documentation.
  • Teach patients/families/caregivers about a patient’s medical condition, status, treatment plan, medications, possible side effects, and follow‑up measures. Translate complex medical terminology to ensure complete knowledge.
  • Complete extra discharge documentation, education, and discharge care for special patient populations to ensure compliance with core measures for discharge.
  • Ensure patients/families/caregivers receive copies of discharge paperwork and any other documentation needed, such as referrals for ongoing care and educational materials.
  • Coordinate with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.
  • Demonstrate a time‑conscious awareness and ability to prioritize, distinguishing between low and high‑priority activities.
What Qualifications You Will Need
  • Advanced Cardiac Life Support (ACLS) must be obtained within 60 months of employment start date.
  • Basic Cardiac Life Support (BCLS) must be obtained within 30 days of employment start date.
  • Registered Nurse license.
  • Associate Degree, Registered Nurse Diploma, or Bachelor’s Degree.

Parkridge Medical Center is a 270+ bed hospital located near downtown Chattanooga. We offer comprehensive diagnostic, medical, and surgical services, including cardiovascular surgery, cancer treatment, intensive care, orthopedic and spine surgery, rehabilitation, neurosurgery, OB‑GYN, and a level III Neonatal Intensive Care Unit. Behavioral health treatment centers for children and adults are also available.
