Beacon Home Health is seeking an RN Case Manager in Clarksville, TN for a full-time position within our Home Health team. The RN will deliver and continually evaluate the plan of care, perform patient visits, and coordinate with physicians and families to ensure quality care.
Requirements include 1 year of clinical RN experience, an active RN license in the state, and CPR certification. Must be able to commute to multiple patient residences in the assigned area.
