Pay: $28 – $32 per hour (based on experience & case)

Setting: In-Home Private Duty Nursing

Interim Health Care of Montgomery County is seeking a dependable and compassionate Licensed Practical Nurse (LPN) to provide one-on-one care in a home setting.

This role is ideal for nurses who prefer consistent patient assignments
, lower patient ratios, and the ability to build meaningful relationships while delivering skilled nursing care.

• 1:1 Patient Care – No facility overload or high patient ratios
• Flexible Scheduling – Day, night, and weekend shifts available
• Consistent Hours – Long-term cases with stable schedules

• Provide direct skilled nursing care in the patient’s home
• Monitor and document vital signs, patient status, and changes in condition
• Administer medications and treatments as ordered
• Assist with ADLs (as appropriate to care plan)
• Perform trach care, G-tube care, wound care
  , or other skilled tasks as needed
• Communicate effectively with RN supervisors, physicians, and care team
• Maintain accurate, timely documentation in the EMR

• Active LPN license in Tennessee
• Current CPR certification
• Experience in home health, private duty, or long-term care preferred
• Strong clinical and observation skills
• Reliable transportation and ability to travel to assigned case

• Pediatric or adult ventilator/trach experience
• G-tube and enteral feeding management
• Wound care and chronic condition management

• Ability to assist with patient mobility and transfers
• Ability to perform extended patient care shifts (8–12 hours typical)
