Position SummaryThe Case Manager is responsible for assessing participant needs, coordinating and implementing individualized service plans, and advocating to ensure access to necessary services and resources. Through collaboration, care coordination, and community engagement, the Case Manager helps participants achieve recovery goals, improve overall well-being, and increase independence.
This position serves as a key link between participants, families, healthcare providers, behavioral health professionals, and community resources to ensure individuals receive the support they need to thrive.
Essential Responsibilities Participant Assessment & Service Planning- Assess participant strengths, needs, goals, and barriers to identify appropriate services and supports.
- Develop, implement, and monitor individualized service and treatment plans.
- Support participants in achieving recovery, wellness, and life goals.
- Conduct screenings and assessments to identify service needs and opportunities for intervention.
- Monitor participant progress and adjust service plans as needed.
- Coordinate services among healthcare providers, behavioral health professionals, community agencies, and support systems.
- Facilitate referrals and connect participants to appropriate treatment programs, community resources, and support services.
- Assist participants in obtaining benefits and resources, including:
- Medicaid and healthcare benefits
- Housing assistance
- Employment resources
- Transportation services
- Community support programs
- Help participants navigate complex systems and remove barriers to accessing care.
- Advocate for participants to ensure access to needed services and resources.
- Promote self-advocacy, empowerment, and participant choice.
- Provide community-based support services focused on recovery and independence.
- Collaborate closely with participants and their support systems to encourage positive outcomes.
- Work collaboratively with multidisciplinary teams to coordinate care and services.
- Develop and maintain productive relationships with community agencies and referral partners.
- Participate in case conferences, treatment planning meetings, and team discussions.
- Identify service gaps and recommend improvements to enhance participant outcomes.
- Complete accurate and timely assessments, case notes, service plans, and progress documentation.
- Maintain compliance with organizational policies, funding requirements, licensing standards, and accreditation regulations.
- Protect participant confidentiality and adhere to all HIPAA requirements.
- Maintain organized records and documentation in accordance with agency standards.
- Support program operations and administrative case management functions.
- Attend required meetings, trainings, and professional development opportunities.
- Assist with quality improvement initiatives and program development activities as needed.
Required Qualifications
- Bachelor's Degree in Human Services or a related field.
- Valid driver's license and reliable transportation.
- Intermediate computer skills, including proficiency with Microsoft Office applications.
- Ability to work effectively in both office and community-based settings.
- One (1) year of case management experience.
- Two (2) years of experience supporting individuals in:
- Behavioral health services
