Virtua is seeking a Hospice Registered Nurse Case Manager to join our team serving the New Jersey hospice service area. You will provide compassionate, patient- and family-centered care in the home setting, coordinating with physicians and an interdisciplinary team.
Responsibilities include assessing patients, educating families on symptom management and prognosis, and developing individualized care plans. Bi-weekly team meetings, on-call, and weekend shifts may be required statewide travel.
