Weekend Wound Care Nurse Summary / Objective
In keeping with our organization’s goal of improving the lives of the Residents we serve, the Wound/Treatment Nurse plays a critical role in providing superior customer service and nursing care to all Residents. The Wound Nurse is responsible for performing skin treatments for all Residents under their care. This position will obtain treatment orders from attending physicians and assist with modifying the treatment regimen in accordance with established policies and procedures.
The Treatment Nurse will provide supervision of staff and will safeguard the health, safety and welfare of all Guests under their care by following applicable laws, regulations, and established nursing policies and procedures.

Daily Duties- Update skin sheets every 7 days, choosing a particular day and ensuring update by close of business on that day.
- Unit Managers to update care plans weekly for pressure and non‑pressure areas; care plans updated every week by Friday.
- Submit Wound Report to VP of Clinical Services by Thursday; the report should show the % of Center Acquired Pressure Ulcers, with a goal of 3% or less.
- Complete treatments for Vascular, Surgical, and pressure ulcers of stage 2 or greater Monday‑Friday.
- Skin Tears, stage 1, to be treated by the Charge Nurse Monday‑Friday with skin sheet completion weekly. Unit Manager and Wound Nurse must ensure completion; they should remind the Charge Nurse and verify the next day.
- Prevent wounds through daily education of staff: reminders for T&P residents (in wheelchairs, geri‑chairs, or in bed), toileting, and incontinent care. Adopt a proactive vs. reactive approach to wound care and hold nurses and CNAs accountable for preventative maintenance. This team effort helps heal wounds and prevents new ones.
- Change treatment within 2 weeks if wounds worsen, in consultation with the Wound physician and PCP at the facility.
- Complete all preventative items for residents at risk of Center Acquired Pressure Ulcers: T&P orders, preventive barrier cream (preferably with zinc), pressure‑relieving device for chairs, gel cushion for every resident, Braden assessments on new admissions, re‑admissions, quarterly, and after any change in condition. Apply a specialty mattress if Braden scores are low; place at‑risk care plans for all residents.
- Consult with the Nursing team regarding assessment evaluations and help develop skin‑care treatment plans.
- Initiate requests for consultation or referral as needed.
- Assess residents upon admission, discriminate between normal and abnormal findings, and recognize when to refer a resident to a physician for evaluation, supervision, or directions.
- Assume a Unit Nurse role when directed.
- Supervise CNAs and other subordinate staff, checking their work to verify that assignments are completed.
- Administer medications within the scope of practice of an RN or LP‑N; maintain a current narcotics report.
- Conduct direct contemporaneous charting during your shift and prepare detailed monthly evaluations of each resident that reflect their condition and progress.
