Position: Employment Support Professional, LTSS

Job Summary

The Employment Support Professional, LTSS is responsible for supporting people to achieve personal dreams and goals by obtaining and maintaining competitive employment within their community.

Scheduled Hours & Compensation

Weekly hours: 7 (Saturday & Sunday, 12:30pm‑4:00pm). Pay Range Minimum: $15.00.

Qualifications
  • High School Diploma (Required)
  • Valid U.S. Driver’s License
  • Experience working with adults with a mental health diagnosis and/or developmental disabilities (Not Required)
Responsibilities
  • Help make dreams take flight through personal outcome measures, skills training, and the development of individualized dream plans.
  • Identify organizations to provide volunteer and/or employment opportunities and nurture relationships.
  • Support individuals in setting long‑term employment goals, reviewing work history, strengths, culture, justice involvement, etc.
  • Complete and update a career profile for each individual with information from the person, mental health practitioners, and with permission from family members or past employers.
  • Assist individuals in learning how benefits will be affected by earned income.
  • Discuss disclosure of psychiatric status to employers, including costs, benefits, and information to disclose.
  • Develop holistic, integrated interventions.
  • Identify services, resources, and accommodations for individuals who are Deaf and Hard of Hearing, those with Limited English Proficiency, and those who are blind or visually impaired.
  • Coordinate services and work with the individual and team to develop an individual employment plan and updates.
  • Perform Job Development activities directed toward positions individualized to each person following IPS principles.
  • Develop and implement task analysis for each individual receiving services.
  • Ensure the individual performs job tasks to the employer’s satisfaction, including performing job duties as necessary.
  • Provide individualized follow‑along supports to maintain employment.
  • Provide education and support to employers, including negotiating job accommodations and follow‑along contact.
  • Provide outreach when appointments are missed, using various methods to identify barriers.
  • Spend at least 65% of scheduled work hours in the community performing job duties.
  • Help with career development, finding desirable jobs, requesting raises/promotions, and accessing education and training programs.
  • Meet individual placement and billing goals set by supervisor.
  • Demonstrate flexibility to alter schedule to meet demand.
  • Complete required documentation timely.
  • Communicate needs and progress to all appropriate team members.
  • Assist clients with accessing transportation and community resources.
  • Collaborate with outside behavioral health providers, families, natural supports, housing, transportation, and other community service providers.
  • Attend and actively participate in meetings and training as required.
  • Maintain certification in all agency, state, and federal training requirements.
  • Demonstrate knowledge of and compliance with agency policies, procedures, and statutes.
  • Demonstrate knowledge of emergency procedures and assist in crisis situations.
  • Travel and driving may be required.
