Rocky Mountain Care – Clearfield is seeking an energetic and dynamic individual to join our skilled nursing facility as a
** Full‑time Resident Advocate/Discharge Planner**. The position is located in Clearfield, UT 84015 and is a full‑time/part‑time role within the Health Care category.

Responsibilities
  • Provide social services to meet the needs of residents and families.
  • Engage interdisciplinary team members, patients and family members, offering emotional support for end‑of‑life care when appropriate.
  • Attend all interdisciplinary team (IDT) and mandatory meetings, communicating effectively with team members.
  • Complete all required documentation in compliance with rules and regulations, including documentation of communication skills and activities with patients such as assessment of emotional factors, counseling for long‑range planning and decision‑making, community resource planning, short‑term therapy, etc.
  • Provide timely and effective discharge planning.
  • Perform other duties as assigned.
Position Requirements
  • Excellent punctuality and attendance.
  • Experience in nursing home environments (a must).
  • Experience in discharge planning.
  • Ability to multitask and meet deadlines.
  • Solid knowledge of a clinical environment.
  • Patience and professionalism.
Required Experience
  • Resident Advocate with prior experience in discharge planning.
Qualifications
  • Minimum of a high‑school diploma or equivalent.
