Assignment Dates: April 27, 2026 – July 5, 2026.
Schedule: Day Shift | 40 hours/week.

As an RN Care Manager in Home Health
, you’ll serve as the vital link between patients, families, and the healthcare team—ensuring coordinated, high-quality care in the comfort of patients’ homes.

Important: This is HOME HEALTH (not hospital) — prior experience is required
.

• Lead and coordinate patient care across a multidisciplinary team
• Develop, implement, and adjust individualized care plans
• Collaborate with physicians and healthcare providers to optimize outcomes
• Guide patients and families through care decisions and transitions
• Monitor treatments, diagnostics, and resource utilization
• Support complex cases, including chronic conditions and high-utilization patients
• Ensure compliance with regulatory and accreditation standards

Required:

• Active Registered Nurse (RN) license (California or eligible)
• Home Health experience (MUST HAVE)
• HCHB (Homecare Homebase) software experience (REQUIRED)
• OASIS documentation experience (REQUIRED)
• BLS certification

Preferred:

• Bachelor’s Degree in Nursing (BSN)
• Case management experience or certification

All associates must meet vaccination requirements, including:

• COVID-19 (required in CA)
• MMR and other standard immunizations (Medical/religious exemptions may apply)
