Schedule: Day Shift | 40 hours/week

As an RN Care Manager in Home Health, you’ll serve as the vital link between patients, families, and the healthcare team—ensuring coordinated, high‑quality care in the comfort of patients’ homes.

Important: This is HOME HEALTH (not hospital) – prior experience is required.

• Lead and coordinate patient care across a multidisciplinary team
• Develop, implement, and adjust individualized care plans
• Collaborate with physicians and healthcare providers to optimize outcomes
• Guide patients and families through care decisions and transitions
• Monitor treatments, diagnostics, and resource utilization
• Support complex cases including chronic conditions and high‑utilization patients
• Ensure compliance with regulatory and accreditation standards

• Active Registered Nurse (RN) license (California or eligible)
• Home Health experience (MUST HAVE)
• HCHB (Homecare Homebase) software experience (REQUIRED)
• OASIS documentation experience (REQUIRED)
• BLS certification

• Bachelor’s Degree in Nursing (BSN)
• Case management experience or certification

• COVID-19 vaccination (required in CA)
• Influenza (seasonal)
• MMR and other standard immunizations (Medical/religious exemptions may apply.)
