Key Responsibilities:
Engages members telephonically to identify barriers to care related to Social Determinants of Health (SDOH) Facilitates the development of personalized care plans by identifying members' strengths, needs, and goals Coordinates with the member's health care team to enhance advocacy and communication regarding care delivery
Qualifications:
Active PA Registered Nurse (BCBA preferred) LCSW, LSW or LPC required Minimum of 3-5 years post licensure experience in an integrated care setting or health care environment Strong understanding of Social Determinants of Health and Motivational Interviewing techniques Ability to work independently and resolve complex behavioral, medical, and psychosocial issues Strong computer skills and experience with Microsoft Office Suite
